Identification, citizenship, and age proof
Most insurers will just require one that shows your date of birth and identification, as well as another that certifies your citizenship such as a passport:
A valid driver's license or photo identification. Some health insurance companies might require proof of residency. You can give them one of the following options:
A mortgage payment or property tax report serves as confirmation of your present residence for homeowners.
An apartment lease contract, rent receipt, or signed letter from your landlord if you're a tenant.

Your return postal address on a postmarked envelope
Evidence of current or future earnings
Because you may be qualified for a tax credit under the Affordable Care Act to help you pay insurance premiums, having proof of income is critical while shopping for health insurance.

Any of the following documents will suffice:
Payroll stubs
A signed letter on company letterhead from your employer
An income tax return from the federal or state government
Accounts receivable and/or payroll records
If you're self-employed, you'll need to submit a tax return as well as an earning record (as shown on your 1099 form) and/or company costs.
If you're unemployed, you should:
Proof of earnings (unemployment benefits award letter/monthly statements)
Any official correspondence from the Department of Labor in the United States
You may be asked to furnish your Social Security number by your health insurance carrier. Payments are considered income, thus an awards letter, statement of yearly benefits, or any other correspondence from the Social Security Administration can be used as proof of income.
